About This Condition
Swelling or edema refers to an abnormal, perceptible increase in interstitial fluid volume in tissues, often seen as puffiness or enlargement of the affected area. It may be localized (for example a single limb or joint) or generalized (anasarca) and can be acute, recurrent, or chronic. Patients may notice tight shoes or rings, sock indentations, facial swelling, abdominal distension, or new difficulty bending fingers or ankles.
Edema arises when the balance between capillary filtration and lymphatic drainage is disrupted, due to changes in hydrostatic pressure, oncotic pressure, capillary permeability, or lymphatic flow. Common systemic causes include heart failure, kidney disease, liver disease, and certain medications. Localized causes include venous insufficiency, deep venous thrombosis, lymphatic obstruction, local inflammation, and trauma. In some individuals, edema reflects a mixture of systemic and local factors plus lifestyle contributors such as high sodium intake and prolonged sitting or standing.
Clinical evaluation aims to determine whether edema is localized or generalized; acute or chronic; pitting or non-pitting; and symmetric or asymmetric, and to identify associated symptoms such as dyspnea, orthopnea, abdominal distension, pain, redness, or skin changes. This distinction helps narrow the differential between cardiac, renal, hepatic, venous, lymphatic, inflammatory, and medication-induced causes. Management targets the underlying condition and may include sodium restriction, compression, elevation, diuretics when appropriate, and lymphatic or venous support strategies.
